These plans, such as Medicare, Blue Cross/Blue Shield, Geisinger, Health America, UPMC, will cover office visits and tests at our office for medically related eye problems.

NVA - Eye Exams, glasses or contact lenses.
VBA - Eye exams, glasses or contact lenses.
VSP - Eye exams, glasses or contact lenses.
UPMC Health - Eye examinations and medical eye care.
UPMC - UPMC for You, UPMC for Kids, UPMC for Life
Medicare - Eye examinations and medical eye care.
Medicaid - Eye exams. Glasses for children under 21. Medical eye care.
Geisinger - Eye exams are usually covered. Materials depend on plan. Medical eye care.
Geisinger Gold - Eye exams yearly, glasses every 2 years. Medical eye care.
Superior Vision - Aetna Better Health. Eye exam; glasses or contacts.
Blue Chip - Eye examinations, eyeglasses.

If you choose to see us as an out of network provider, you will usually be able to receive some reimbursement for the examination, as well as some for materials you may purchase (glasses or contact lenses).
A claim form will need to be submitted to your insurance company. We will be happy to fill out your claim form for you. All we ask is for you to bring along the required forms with you to your appointment. These forms are usually available online at your insurance provider's website.
